Maternity Admission Information

Please complete your maternity admission form provided to you by your Obstetrician or download the PDF on this page by clicking on the link. You then need to complete and return the form by hand to the hospital reception team.  Alternatively you can book online by going to our hospital info/patient admission page. In conjunction with your online admission form submission you will need to submit your Obstetricians consent form to tgcmaternitybookings@acurio.com.au 

Once your booking form is received, you will be contacted by one of The George Centre’s midwives to attend a midwife booking appointment. You can expect to be contacted within 2-3 business days from when you submit your Admission form, but please don’t hesitate to contact us if you have any queries while you are waiting for our team to get in touch.
